Repairs to be completed as per the estimate. If additional parts and/or repairs are required, authorization for these supplementary items is granted with the signing of this document. When applicable a pre and post diagnostic vehicle scan may be required to meet Manufacturer's repair standards. I hereby authorize the above repair work to be done along with any necessary materials. All Makes Collision Centre employees may operate the vehicle for purposes of testing, inspection or delivery as required. All Makes Collision Centre will not be held responsible for loss or damage to the vehicle or articles left in the vehicle in case of fire, theft, accident or any other cause created that is beyond our control.
